Who is the problem?

When the spoken word is incorrect on a TV show, Who is at fault? The script writer? The actor? The director?

On the last episode of the tv series NCIS LA, the character Hetty Lange, played by Linda Hunt, uttered the phrase “I’ll find out momentarily.” As we should know by now, the word “momentarily” means “for a moment,” not “in a moment.”

Example: He paused momentarily before entering the room.
